FSS- baixa estatura familiar ocorre quando uma criança, infantil, juvenil, adolescente tem altura abaixo do terceiro percentil devido a uma tendência genética para a baixa estatura em sua família. Uma criança, infantil, juvenil, adolescente com FSS- baixa estatura familiar geralmente atinge a estatura adulta de acordo com seus antecedentes familiares. CGD-atraso constitucional do crescimento ocorre quando uma criança é mais baixa em altura (baixa estatura) do que seria esperado por ela ou por sua origem genética e sem causa médica determinável da baixa estatura. Muitas vezes, as crianças com CGD-atraso constitucional do crescimento experimenta um atraso no início da puberdade e geralmente chega à altura normal ou quase normal de um adulto. A critical analysis of longitudinal or linear low stature takes the anguish of the parents to a complex situation, to know the final height projection of children in adulthood. Endocrinologists and neuroendocrinologists are in a position of having to offer a number, taken from formulas, which gives results are not always accurate. One factor is the medium and long term and depends on the perseverance of the parents and the patient such as those related to the environment, nutrition, bio-psychosocial, physical exercises that add abstract variants, independent of organic. The methods of final height are flawed and based on parameters that are difficult to quantify, e.g., bone age that are not always accurate, regardless of the protocol used as standards. Moreover, the evaluation of various therapeutic modalities based on the modification of the prediction of final height to more beneficial treatment effect, which can cause some misconceptions regarding the analysis of the therapy in question.
It's relentless commitment to therapeutic results in this area when we have not made a pre-natal, neonatal outcome, detailed physical examination, careful laboratory and instrumental tests without considering the frightening evolution of science in this and other related areas which requires us to review and continuous updating without considering protests that books can take up to 10 years (with exceptions) to bring us new information, which does not happen with magazines, handouts, observatories publications, meetings, conferences etc., the detail is that these factors have an exorbitant cost to the clinical professional, but is probably the only way to upgrade. Therefore we must make the differential diagnosis more closely analytically with the tools possible, so that the result of our intervention to parents and give the patient a sense of responsibility from each of us, helping to design a good quality of life with the possible development of each stage is indicated and useful for our intervention. There are several causes of short stature. The most common causes are familial short stature (FSS) and constitutional growth delay (CGD). FSS-familial short stature occurs when an infant, child and adolescent, have height below the third percentile due to a genetic tendency to short stature in their family. A child, infant and adolescent with FSS- familial short stature usually reaches adult height according to their family background. CGD-constitutional delay of growth occurs when a child is lower in height (short stature) than would be expected by it or on its genetic origin and without ascertainable medical cause of short stature can be found. Often, children with CGD-constitutional delay of growth experience a delayed onset of puberty and usually get normal or near normal height of an adult.
Either FSS-familial short stature or CGD-constitutional delay of growth is considered to be due to causes and studies. Some researchers suggest using GH-Growth Hormone to optimize the target of genetically height expected; medically determinable in most cases. Since it can be difficult to differentiate between these two conditions, the term isolated short stature (ISS), is often used as a synonym for both FSS-familial short stature and CGD-constitutional delay of growth. Medically determinable causes of short stature include abnormalities in GH-growth hormone, such as decreased growth hormone production and decreased response to GH-growth hormone. Other endocrine abnormalities such as hypothyroidism and Cushing's disease may lead to short stature and a variety of genetic diseases, including chromosomal, metabolic, and single gene disorders may also result in short stature. Skeletal dysplasias are genetic disorders that result in abnormal formation of part or the entire skeleton. Not all skeletal dysplasia will result in short stature in a child, infant, adolescent and teenager. Skeletal dysplasias most likely to lead to short stature are those that involve the formation and growth of long bones and/or spine. The presence of a chronic illness in a child has been known to be a risk factor for reduced growth in varying degrees. However, the underlying cause of decreased growth has not been determined for all chronic diseases.
